The Path of Care

Every member's journey is unique, but our structured approach ensures no one falls through the cracks.

Step 1

Referral & Intake

Hospital teams, shelters, and community providers refer members directly. Our intake coordinators assess medical acuity, behavioral health needs, and housing status within 24 hours.

Step 2

Medical Stabilization

Members rest and stabilize in a calm, non-institutional setting with a structured daily routine. Staff support supervised self-administration of medications and coordinate closely with each member's own medical providers to ensure continuity of care.

Step 3

Behavioral Health Support

We connect members, with their consent, to behavioral health, primary care, and substance-use services in the community.

Step 4

Care Coordination

Our team connects members with Medicaid enrollment, housing navigators, primary care providers, and community-based services to ensure continuity beyond our doors.

Step 5

Discharge & Transition

No one leaves without a plan. We coordinate warm hand-offs to housing, outpatient care, and peer-support networks so recovery continues after Mana Respite.
